Finding Our Office

Here's how to find our office. We're on the second floor of the Jakadofsky Building on the UF Health Flagler Hospital campus in St. Augustine.

Driving Directions (from US-1)

  1. From US-1, turn onto Health Park Blvd.
  2. Follow Health Park Blvd all the way down to the stop sign.
  3. Turn right.
  4. At the next stop sign, turn left.
  5. Turn right to enter Parking Lot D.

Parking

Park in Lot D. It sits directly behind our building, 201 Health Park Blvd (the Jakadofsky Building).

  1. If Lot D is full, the next closest option is Lot E, reached from Sergeant Tutten Drive.
  2. If Lot E is also full, use Lot C as the final option.

Entering the Building

Once you're parked, there are two ways up to our suite on the 2nd floor:

  1. Stairs — the stairwell on the left side of the building brings you straight up to our suite.
  2. Elevator — walk to the middle of the building, take the elevator to the 2nd floor, turn left, and follow the signs for Whole Health Oncology.

Arrival & Parking

Please plan to arrive 15 minutes before your scheduled appointment time. If you would like to complete your registration paperwork on an iPad in our office, please arrive a minimum of 30 minutes early.

Parking Lot D is closest to our building — please check there first. If no space is available, the next best option is Parking Lot C.

Registration

You can complete your registration online before your visit. We will send you a link to your patient portal, which will be sent separately from this email.

If you do not receive a link to your patient portal before your appointment, please call our office so we can send it to you.

Completing your registration in advance allows us to get you checked in quickly and spend more time focused on your care.

Insurance & Billing

Whole Health Oncology is an out-of-network practice. We want to be straightforward with you about what that means:

Your out-of-pocket cost for the consultation is the same as it would be if Dr. Bashir were in-network. There are no additional bills for your consultation visit.

Frequently Asked Questions

Are you in-network with my insurance?

No. Whole Health Oncology is an out-of-network practice.

How much is my consultation?

Your out-of-pocket cost for the consultation is the same as it would be if Dr. Bashir were in-network.

Will I receive additional bills for the consultation?

No. The consultation fee covers your consultation visit in full.

What if Dr. Bashir recommends surgery?

If surgery or another procedure is recommended, our team will review the insurance and financial process with you before anything is scheduled.
